Projects for Assistance in Transition from Homelessness (PATH) serves people living with serious mental illness who are experiencing homelessness or are at imminent risk of becoming homeless. Outreach is focused on people who are living on the streets or in shelters. People with co-occurring mental illness and substance use disorder can also qualify for the program.
