Trinity Care Homes & Transit Services LLC is a
provider of long-term Supportive Living Housing &
Transportation Services.
Our Semi-Independent living arrangements provide
a shared home living space for 3 adults.
Pricing is all-inclusive for private room and some
services. Medication assistance and transit services
are provided to individuals needing non-emergency
medical transportation to doctor’s appointments and
the pharmacy at an additional fee.
We provide weekly visits to check on adequate ADL
care, completion of house chores, and overall wellbeing. We encourage daily success by supporting
individuals with personal appearances, adequate
nutrition, and keeping appointments to their
primary care physician.
